be7fe67145 Fix critical publish() bug, address review findings
CRITICAL: createPubSub.publish() was dispatching CustomEvent with
just the event type (e.g. 'call.responded') instead of the composite
topic string ('call.responded:uuid-123'). This broke all adapters
that rely on topic-scoped dispatch — Redis subscribe/publish
channels didn't match, and WS server fan-out routing would fail.
Fixed to dispatch with the full type:id composite.

Other fixes:
- Add __ prefix runtime guard in publish() (reserved for control)
- Add Redis barrel re-export to src/index.ts (ADR-002 compliance)
- Clarify WS server: adapter's onclose calls removeConnection
  internally; user doesn't need to
- WS client: document null callback no-op, removeEventListener
  edge cases (unregistered callback, null callback)
- WS server: document dispatchEvent always returns true
- Redis spec: document in-flight message edge case after unsubscribe
- Worker adapter: rename createMainThreadEventTarget to
  createWorkerThreadEventTarget, createWorkerEventTarget to
  createWorkerHostEventTarget (fix inverted naming)
- api-surface.md: add PubSub.publish() section documenting the
  type:id composite and __ guard

e60f0a1aa0 Mark Iroh adapters as deferred pending fork of iroh-ts
Update peer dep from @rayhanadev/iroh to @alkdev/iroh. Document
platform strategy: Linux native + WASM fallback, Windows/macOS
deferred. Fix outdated single-import reference in iroh-transport.md
to split spoke/hub imports. Resolve binding stability and NAPI/Deno
R&D items as covered by the fork path.

bc0c2589c7 Resolve WebSocket event target open questions, add subscription control protocol
- Resolve OQ1: WS server accepts raw WebSocket instances via
  addConnection/removeConnection (framework-agnostic, not coupled to
  Hono/Express/Bun/Deno)
- Resolve OQ2: Backpressure handled by disconnecting slow consumers at
  configurable threshold (default 1MB), with onBackpressure callback
  for observability
- Resolve OQ3: Topic-based fan-out with subscription tracking instead
  of broadcast-all; spokes send __subscribe/__unsubscribe control
  events; direct messaging via 'direct:' topic pattern

Add ADR-003 for subscription control protocol decision. Update all
fan-out adapters (WS server, Iroh hub) and spoke adapters (WS client,
Iroh spoke) with subscription tracking/forwarding. Fix routing key
ambiguity (full topic string, not event type alone). Add error
handling, composition, and reserved type sections. Clarify Worker as
symmetric-only.

371dabc20d Add per-adapter architecture docs in event-targets/ directory
- Create docs/architecture/event-targets/ with individual specs:
  in-process, redis, websocket-client, websocket-server,
  worker, iroh-spoke, iroh-hub
- Update event-targets.md to serve as index with topology model
  (symmetric vs fan-out) and adapter status table
- Update architecture.md index to reference new directory
